src/include: Add CPER definitions
Add definitions from the UEFI appendix on Common Platform Error Record (appx. N in revision 2.7-A). The structures and fields defined are the minimum required for generating ACPI Boot Error Record data in a subsequent patch. BUG=b:65446699 TEST=inspect BERT region, and dmesg, on full patch stack. Use test data plus a failing Grunt system.
