- The 4.19 release notes included a list of outstanding issues formatted
as a markdown table, which is not supported by Recommonmark. Reformat
as an embedded reStructuredText table.
- The table of boards supported on the 4.18 branch did not include row
separators causing all rows to be rendered in a single row of cells.
- Technotes/console.md had a typo in the rST table formatting which
generated warnings in the Sphinx build, causing the table to not be
rendered in the resulting html.

Fix the exclusion path for lcov; it should exclude the directory
with source code, not object files.
Use the COV environment variable to
* control whether we build for coverage or not
* select the output directory
Add a separate target for generating the report, so we can get a
report for all of the tests together or just a single test.
Add documentation.

The manifest example has to be marked as code. The Manifest parsing
section header should be L2 and finally # is a special character in
markdown.

It may be time to revisit the way we define our image layouts now that
there are new fmap schemes for new vboot uses. The approach outlined in
this document may or may not be the right one, but it's something we
have, so let's discuss.

* integrate feedback to the doc and on the mailing list back then
